Splenectomy is helpful to correct unbalance between oxidation and antioxidation in patients with Liver cirrhosis

摘要: 探讨脾切除对肝硬化患者氧化和抗氧化系统的影响及其临床意义。测定 2 8例肝硬化脾切除患者、32例肝硬化非脾切除患者和 6 0例正常人血浆维生素C(P -VitC) ,维生素E(P -VitE) ,β-胡萝卜素 (P - β-CAR)和一氧化氮 (P -NO)含量 ,红细胞超氧化物歧化酶 (E -SOD)、过氧化氢酶 (E -CAT)和谷胱甘肽过氧化物酶 (E -GSH -Px)活力 ,血浆及红细胞过氧化脂质 (P -LPO ,E -LPO)水平的变化。肝硬化非脾切除组、脾切除组和对照组的P -VitC、P -VitE、P - β -CAR、E -SOD、E -CAT和E -GSH -Px水平依次升高 (均P <0 0 0 1) ;三组的P -NO、P -LPO和E -LPO含量依次降低 (均P <0 0 0 1)。除了脾切除组和对照组E -SOD差异无统计学意义外 ,三组之间其他检测值的两两比较均有统计学意义。脾切除有利于纠正肝硬化患者体内一氧化氮代谢异常、氧化和抗氧化的失衡。Abstract: To assess the effects of splenectomy in liver cirrhosis on oxidant and antioxidant system and its clinical significance. The plasma levels of vitamin E (P-Vit E) , vitamin C (P-Vit C) , beta-carotene (P-β-CAR) , lipoperoxides (P-LPO) and nitric oxide (P-NO) , the erythrocyte levels of superoxide dismutase (E-SOD) , catalase (E-CAT) , glutathione peroxidase (E-GSH-Px) and E-LPO in cirrhosis patients were measured in 28 liver cirrhosis patients with splenectomy, 32 without splenectomy, 60 sex-and age-matched subjects from healthy people as the control group. It showed that the levels of P-Vit E, P-β-CAR and P-Vit C as well as activity of E-SOD, E-CAT and E-GSH-Px gradually increased in cirrhosis without splenectomy group, in cirrhosis with splenectomy group and in normal control group (all P <0 001) ; While the contents of P-LPO, E-LPO and P-NO gradually decreased (all P <0 001) . Multiple comparisons showed there were statistical difference among three groups ( P <0 001, P <0 01 or P <0 05) except the levels of E-SOD between cirrhosis with splenectomy group and normal control group. Splenectomy is helpful to correct abnormal metabolism of nitric oxide and unbalance between oxidation and antioxidation in patients with liver cirrhosis.
