A prospective follow-up study on the HBV-infected cases with anti-HBs positive sera.

摘要: 目的观察乙肝表面抗体（抗-HBs）阳性HBV感染者5年疾病演变。方法对62例抗-HBs阳性者检测5年前后血清乙肝病毒标志物（HBVM）、HBVDNA水平及观察临床演变。结果5年后肝硬化、肝癌发生率分别11.3%、4.8%,死亡5例见于123、12阳性模式组,123、12、245阳性模式5年后仍有48.1%、66.7%、61.1%保持原模式不变,临床类型构成比有显著性差异,5年后抗-HBs和HBVDNA水平总趋势下降,123模式HBVDNA定量显著下降,12模式抗—HBs水平显著下降。结论HBVDNA和血清抗-HBs水平高低是预示疾病转归的重要指标。Abstract: Objective to observe the disease prognosis in HBV-infected cases with anti-HBs positive after five years.Methods HBV markers were examined by abbott and HBVDNA was detected by polymerase chain reaction from 62 cases sera with anti-HBs before and after five years.Results Five years later, the incidence of liver cirrhosis and hepata carcinoma were 11.3%, 4.8% respectively.Five patients died in HBsAg、anti-HBs、HBeAg positive group and HBsAg、anti-HBs positive group.HBsAg、anti-HBs、HBeAg positive group, HBsAg、anti-HBs positive group and anti-HBs、anti-HBc、anti-HBc positive group could keep primary condition 48.1%、66.7%、61.1%respectively after five years and there were significant difference in clinical constituent ration.Levels of anti-HBs and HBVDNA were decreased generally after five years.Level of HBVDNA was markedly declined in HBsAg、anti-HBs、HBeAg positive group and anti-HBs was obviously dropped in HBsAg、antiHBs positive group.Conclusion Serum levels of HBVDNA and anti-HBs could be important predictor for disease advance.
