Application of minimally invasive integrated traditional Chinese and Western medicine therapy based on the SELECT concept in the diagnosis and treatment of hepatobiliary and pancreatic diseases

肝胆胰疾病是消化系统常见病、多发病,部分肝胆胰疾病诊治棘手,并发症发生率高。随着微创诊疗设备和器械的研发,各种内镜腹腔镜技术的开展,使大多数肝胆胰疾病通过微创技术得到诊治。中医药在肝胆胰围手术期有着重要的协同作用,能够有效加速患者康复。笔者领衔的胆胰疾病中西医结合治疗中心团队掌握了多种内镜技术和腹腔镜技术,总结了多年来治疗肝胆胰疾病积累的成功经验,提出了SELECT理念(Spyglass+ERCP+Laparoscopy+EUS+Choledochoscopy+Traditional Chinese medicine,SELECT),根据不同的肝胆胰疾病特点,选用最优化的多镜组合微创治疗方式,围手术期结合中医药治疗,实现了中西医结合治疗肝胆胰疾病的微创化、个体化、精准化。
Abstract:Hepatobiliary and pancreatic diseases are common and frequently occurring diseases in the digestive system,and several hepatobiliary and pancreatic diseases are difficult to diagnose and treat,with a high incidence rate of complications. With the development of minimally invasive devices and instruments and the application of various laparoscopic/endoscopic techniques,most hepatobiliary and pancreatic diseases can be diagnosed and treated by minimally invasive techniques. Traditional Chinese medicine plays an important synergistic role during the perioperative period for hepatobiliary and pancreatic diseases and can accelerate the recovery of patients. The team of Liaoning Provincial Center for Integrated Traditional Chinese and Western Medicine Therapy for Biliary and Pancreatic Diseases led by the authors has mastered various laparoscopic and endoscopic techniques and proposed the concept of SELECT( Spyglass,ERCP,Laparoscopy,EUS,Choledochoscopy,and traditional Chinese medicine) by summarizing the successful experience in the treatment of hepatobiliary and pancreatic diseases in recent years. The optimal combination of minimally invasive multi-endoscopic techniques is selected based on the features of different hepatobiliary and pancreatic diseases,and traditional Chinese medicine treatment is also applied in the perioperative period,so as to achieve minimally invasive,individualized,and precise integrated traditional Chinese and Western medicine therapy for hepatobiliary and pancreatic diseases.
