Perioperative coagulation factor Ⅷ replacement therapy for patients with gallbladder stones and hemophilia A: A report of three cases

<正>1病例资料病例1:患者男性,30岁,2018年4月6日因"间断性右上腹疼痛2 d"就诊于本院。血友病A病史20年。查体:右上腹轻压痛,向右肩部放射。腹部超声显示:胆囊腔内多个强回声光团,最大约2.1 cm×1.1 cm。肝胆脾胰多排CT显示:胆囊腔内多个高密度影,肝内胆管及胆总管未见明显扩张。入院诊断为"急性结石性胆囊炎合并血友病A"。凝血因子Ⅷ:C(FⅧ:C)为8.2%,无FⅧ抗体。参考血液科会诊意见结合患者实际情况给予FⅧ替代治疗,手术当天FⅧ制剂按65%替代,术中小心分离并结扎胆囊动脉、胆囊管,使用超声刀分离胆囊,胆囊窝渗血约80 ml,剥离后胆囊床放置凝胶海绵及可吸收再生氧化纤维素(2082)术中止血。术后第1～3天分别按60%、50%、
